Parent/Guardian Consent Form

 All delegates must be at least 18 years old (as of 3 August 2008) to attend the conference on his/her own. All children/youths under the age of 18 (as of 3 August 2008) must be accompanied by an adult (at least 18 years old as of 3 August 2008) who is registered for the conference in order to attend. In addition, each child/youth must complete and submit a Parent / Guardian Consent Form. 

The accompanying child/accompanying youth must bring a copy of the completed form to the conference and carry it at all times. They must be with the main delegate at all times when they are in the conference venue or when they are attending conference-affiliated activities outside of the conference venue and they may be asked to provide official photo identification (e.g., passport, student I.D.) confirming their age.
